White House press secretary Sarah Sanders says President Donald Trump is "not going to apologize for trying to fix our immigration system", as the fall-out continues from Trump's reported use of a vulgarity to describe some African countries.

Rand Paul of Kentucky told NBC's Chuck Todd Sunday that congressional Democrats are making an immigration reform deal impossible by calling President Donald Trump a racist. The man is borderline obese, doesn't exercise, and eats Big Macs.

White House physician Dr. Ronny Jackson has confirmed Trump's health. It's called the Montreal Cognitive Assessment, one of several we use to screen for early signs of dementia and Alzheimer's, and this particular one is considered to be an adequate screening test.

The tabloid-style book Fire and Fury by journalist Michael Wolff, published this month, led to speculation about Trump's mental fitness for office.

Jackson said on Tuesday that Trump asked him to perform the cognitive exam, which the doctor had not planned to do, having deemed it unnecessary.

Later, Trump attempted to clarify matters by tweeting he was a "very stable genius."

Dr Jackson performed a series of tests on the 71-year-old president on Tuesday as part of his first medical exam in office. Jackson said he would increase that dose in an effort to get Trump's so-called "bad" cholesterol, or LDL level, below 120; it now is 143.

"I would say right now on a day-to-day basis, he doesn't have a dedicated, defined exercise program", said Jackson.

The president has a body mass index of 29.9 which places him in the overweight category on the verge of obesity, which is defined as a BMI of 30 or greater.

She then said that the administration now had to deal with other things like a government shutdown before saying the show's opening catch phrase, "Live from NY".

Despite the diet and cholesterol concerns, Jackson stressed that Trump's "cardiac health is excellent".

When a USA secretary of state is forced to give a press conference - as Rex Tillerson did in October - to take the unprecedented step of telling the world he didn't call the president of the United States (his boss), "a f***ing moron", the U.S. is in uncharted territory.

Jackson replied that Trump's heart is "in the excellent category".

"He said, 'Put me down for wanting more Europeans to come to this country".

"The president used tough language in general, as did other congressmen in the room, yes sir", she said later, when Leahy pressed her again.

"I think I need a drink of water", he joked.
